This bulletin describes an oil spill that occurred June 30, 1999 when the M/V SUPER RUBIN was being refueled by the barge MEGHAN 102 on the Columbia River. The probable cause was failure to secure the starboard diesel oil tank valve before loading fuel oil, resulting in the overflow of the diesel oil tank. The bulletin provides a chronology of the incident, lessons learned, and recommendations.
